Public Records Request
Required Information
All requests for public records shall be on a company letterhead stating exactly what records are being requested. The letter should include as much detail as possible to include such information as:
  • Location of incident
  • Time and date of incident
  • The reason why emergency responders were needed

Requests can be submitted online.
